About the Assertive Community Treatment (ACT) Program: ACT is an intensive mobile clinical service for adults with serious and persistent mental illnesses who have histories of non-compliance with traditional outpatient services. ACT services are provided to clients in accordance with the Individual recovery Plan (IRP).

JOB SUMMARY: The Registered Nurse provides medication management and somatic treatment for community-based Assertive Community Treatment team (ACT) to adults who have serious and persistent mental illness. The position provides clinical and crisis services to all consumers served by the ACT team, works with the ACT Director and team leaders to monitor each consumer's clinical status and response to treatment, and directs psychopharmacologic and medical treatment along with the psychiatrist.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Provide a range of nursing and mental health rehabilitation services.
    a. Follow up and participate in the mental health-related medication prescription, administration, and monitoring.
    b. Crisis assessment and intervention.
    c. Symptom assessment, management, and individual supportive therapy.
    d. Assist in the treatment of substance abuse for consumers with co-occurring addictive disorders.
    e. Psychosocial rehabilitation and skill development.
    f. Interpersonal social and interpersonal skill training.
    g. Education, support, and consultation to consumers' families and/or their support system, which is directed exclusively to the well-being and benefit of the
    consumer.
  • Provide an average of 5 billable hours per day.
  • Comply with applicable regulations of the Dept. of Behavioral Health (DBH) and the District of Columbia Board of Nursing.
  • Document all consumer services in Credible (the electronic health record) the same day of service.
  • Attend QI meetings and clinical rounds, as appropriate.

EDUCATION and EXPERIENCE:

  • Bachelor's Degree in Nursing.
  • Licensed as an RN in the District of Columbia.
  • Five years of clinical experience in a health care setting/organization.
  • Relevant experience and expertise in physical health care.
  • Experience treating/caring for consumers with serious mental illness.
  • Driving is required. Must have a valid local license and clean driving record.
  • At least 75% of consumer face-to-face contact will be in the community.

SKILLS and COMPETENCIES:

  • Strong assessment skills, strong clinical judgment and strong advocacy skills.
  • Sound nursing judgment and ability to interface with individuals in crisis.
